Under normal circumstances, people with thinner faces or no dentures and unobstructed nasal passages can choose nasal mask ventilation, but they must keep their mouths closed for breathing. Those with a wide face and mouth breathing should choose to use a mask, and critically ill patients who have weak breathing should use their mouth to breathe more, and should also choose a mask for ventilation. In conclusion, the choice of oxygen mask should be based on the condition and the patient's tolerance. Usually, the nasal mask is the first choice for mild cases, and the oral-nasal mask is replaced when it is ineffective. Mask ventilation is the first choice for severe respiratory failure. The size of the mask should be selected according to the patient's face shape, and there are usually three types large, medium, and small. Those with large faces should choose a large mask. A mask with a suitable size can not only reduce air leakage but also reduce the pressure on the partial skin of the face and increase the success rate of getting on the machine, so be sure to choose a mask with a suitable size.

 

Fixing of oxygen mask

Good fixation can not only reduce air leakage, and ensure the ventilation effect, but also reduce the pressure on the nasal and facial skin.

The connection of the oxygen mask to the patient can be done with 4 headbands and 3 headbands. In theory, compared with the 4-head strap, the 3-head strap is more in line with the mechanical principle, the pressure distribution is the most uniform, the airtightness and comfort are better, and the wearing is more convenient. However, from the experience of use, as long as they are worn properly, both of them can achieve the purpose of good fixation. During immobilization, the patient can take semi-recumbent, sitting, and recumbent positions, but the head, neck, and shoulders should be at the same level, and the head should be slightly tilted back to keep the airway open. When putting on the ventilator, do not fix the mask immediately, first fix the oxygen mask by hand to blow air to the patient, and then gradually reduce the space between the mask and the patient, so that the patient has an adaptation process, and then fix it when the patient accepts it.

When wearing the headband, the upper headband should be fastened flat on the top of the head, and the lower headband should be flatly fastened on the sides of the face, and do not pile up on the back of the neck. The straps on the left and right sides should be pulled back at the same length to avoid air leakage caused by uneven tension caused by uneven force; if the air leaks under the nasal mask, the upper headgear should be loosened slightly, and the lower headgear should be tightened. When wearing a nasal mask, the upper part should reach the root of the nose, the left and right should be close to the nose wing, and the lower part should be at the bottom of the nose (that is, the upper part of the human body). Generally, the smallest model that matches your face shape is the most suitable model. When fixing the nasal (face) mask for patients with special face shapes, a triangular hole can be cut in the middle of a piece of thin cotton with an area slightly larger than the oxygen mask, just enough to allow the nose to pass through and make the oxygen mask in close contact with the skin. Viewed from the side, the mask is just parallel to the patient's face and is a good angle for fixing.
